Most have mentioned, but it should be mentioned again.

    Cannibal Holocaust

This movie is on the top of most top 10 lists for Goriest, as well as most disturbing.

    Directly from Wikipedia
Since its original release, Cannibal Holocaust has been the target of censorship by moral and animal activists. Other than graphic gore, the film contains several scenes of sexual violence and genuine cruelty to animals, issues which find Cannibal Holocaust in the midst of controversy to this day. It has also been claimed that Cannibal Holocaust is banned in over 50 countries, although this has never been verified. In 2006, Entertainment Weekly magazine named Cannibal Holocaust as the 20th most controversial film of all-time.

There are a ton of great recommendations in this thread! Probably one of the best lists I've seen put together on the net. We should compile all of these and show everyone else what a gore list looks like!

Guinna Pig: Flower of Flesh and blood
    Dario Argento: Opera
    Animator/Re-Animator

    Aftermath - 1994 - Aftermath (1994)

    Storyline from IMBD:

"When the others leave for the night, the last mortician begins to fondle the corpses. He quickly moves to the corpse of a young woman who died in a car crash, tearing her clothes off and mutilating her body. He then mounts her and rapes her corpse while taking pictures with a camera and timer. He brings her heart back home to his dog for food."

It's a short film, about 34 minutes long. Very gory, and might be a bit disturbing for some people.

Fair warning, rather gory. And some might find it to be too realistic.

Cannibal Holocaust. Hands Down. It's not fake gore, it's real. I'm pretty sure it's still banned in many countries.
