I love Oldboy, such a great movie with twists and turns throughout the entire film. As far as my vote for most disturbing movie (not sure if this counts) I would add August Underground.
Opening scene begins with fornication between a brother and sister and another man "making love" to a corpse through its gutted up stomach.

I think that the Human Centipede is actually quite legit for physilogical accuracy, but the movie was not so good.

The film Grimm Love is pretty disturbing. If I'd known what it was about I probably wouldn't have watched it. Very uncomfortable... especially if you're a bloke.
