I'll try my best. I just taught myself how to do this, and I'm actually converting a dvd with four audio tracks, nine subtitles tracks, and thirty-two chapters. Normally a dvd has chapters in it already, so no problem there, because a program like handbrake will automatically accept and set those for you. Now for a music video file, you will need to create a xml template for, for instance, handbrake. I guess it would all be dependent on what program you are using? I think the xml template is universal though, so any mkvmerge software should do it.

1.Rip chapters from dvd using ChapterXtractor - Video software and downloads - VideoHelp.com (save in ogm format)
2.Edit the resulting textfile writing the title of each chapters (if you want)
3.Use mkvtoolnix -- Matroska tools for Linux/Unix and Windows to create a .mkv file with chapters
