I have all the important icons that i use regularly on my desktop .
Very organised .
I keep my most used applications as icons in the upper left corner. I keep "Recycle Bin" in the far bottom right, along with several folders directly next to it. I use 'notepad' alot, and often times have a good chunk of text files that fill up my desktop. After a few weeks of that, I'll make a folder called "Text <date>", and then move everything into that folder and place the folder next to the recycle bin, and start over again.
I always use the "Windows Classic" theme. I turn all visualizations off.
I hardly keep any icons on my desktop, launchy will replace them all
Ubuntu by default gives me four desktops. One I use for browsing, one I use for IRC, one I use for torrenting, and the last is a spare for whatever else. I can switch easily between them and I have a customized launcher for my apps.

i have OCD tendencies and i think this is reflected by my desktop.
- i like to save things easily on my computer, so my actual music + movie folders are all on my desktop rather than shortcutted.
- my media folders are labelled - music - , - films+TV -, - pictures -, etc so that when i select my save location they're all grouped together
- my document-based folders are labelled % books, % papers %, % projects %, etc so that they're all grouped together
- periphery of screen, all around, are my folders. i have a background pic with edges which make the folders stand out
- in the centre, i have my daily calendar. the calendar file is hosted on my dropbox, so i can access the calendar on my laptop and iphone
... in fact, i'm getting so excited writing about my desktop that i'm tempted to posted a screenshot of my beautiful desktop!
i know what you all must be thinking --i'm a total fucking nerd. well fuck you cos my mum thinks i'm cool!

Switcher for Windows Vista : expose (that awesome see-all-windows thing for mac, now for windows)
Samurize.com - News : have any widget on your desktop, calendar, system info, weather, rss, etc
News - BB4Win : if you're a hardcore customizer, this is blackbox (from linux) for windows. completely replaces window shell, has a completely customizeable "start menu" that you get by right clicking anywhere, tons of customizable hotkeys, etc.
I went through all of the above software at some point, and they're all good, it's just a preference. Right now I have no desktop icons with tons of hotkeys and hidden docks, and big widgets on my desktop.
On my desktop I have only 4 folders or documents at most, these are usually very important or a reminder for something. All of my other files are kept in their respective folders and categorized alphabetically. I often have many sub folders with my main ones. I'm on mac and the only applications I have in my dock are my most important and frequently used ones. My whole organization system revolves around a very structured and organized approach.
I don't see my desktop that much.. so I really could care less how it's organized. :P
I will, however, use the Windows Task Bar for applications I frequently use.
